I confirm I am aged 18 or over, and that I have read and understood the information about the sessions I am booking.
Not medical treatment
I understand that sessions at Transcend Wellness London are for general wellness, relaxation and lifestyle purposes only. They are not medical treatment and are not intended to diagnose, treat, cure or prevent any medical condition.
My health and medical advice
I confirm that I have sought, or will seek, advice from a qualified healthcare professional if I have any medical condition, am pregnant or breastfeeding, am recovering from surgery, or have any concern about my suitability for a session. I will tell Transcend Wellness London about any change in my health before each visit.
Risks
I understand that some sessions carry minor, usually temporary risks, such as ear discomfort from pressure changes, light-headedness, sensitivity, dizziness or tiredness, and I accept these voluntarily.
Liability
To the extent permitted by law, I agree that Transcend Wellness London, its owners, staff and contractors are not responsible for losses arising from my use of the services or equipment, except where caused by their negligence or breach of contract. Nothing in this waiver limits liability for death or personal injury caused by negligence, for fraud, or for anything else that cannot be limited by law. My statutory rights are not affected.
No guarantees
I understand that experiences and results vary and that no guarantee has been given about results or benefits.
My health information
I give my explicit consent for Transcend Wellness London to collect, store and use the personal and health information I provide, for my safety, the delivery of my sessions and quality control, as described in the Privacy & Cookie Policy.
Emergencies
In an emergency, I authorise Transcend Wellness London to obtain medical help on my behalf and to share necessary information with the clinicians treating me.
Law
This waiver is governed by the law of England and Wales.
